I understand and content to:
I acknowledge that I will receive no compensation for the use of my likeness, voice, or appearance and that all Content shall remain the sole and exclusive property of the Production Companies.
Rights Waiver & Release of Liability
I hereby waive any right to inspect or approve the finished product or any accompanying materials. I release, discharge, and hold harmless Carthan Enterprises, Fabulous Life Productions, Diamond Dance Company, Dr. Jay, and Shanika Kelly Lee, as well as their respective officers, directors, employees, agents, contractors, successors, and assigns, from any and all claims, demands, liabilities, damages, or causes of action arising out of or in connection with the use of my likeness, image, voice, or appearance, including but not limited to claims for defamation, invasion of privacy, right of publicity, or emotional distress.
Minor Participation Consent
If the participant is under the age of 18, this form must be completed and signed by a parent or legal guardian.
